Check to see if you are eligible to earn rewards:
- Look at your member ID card for your CMS#. If your CMS# is listed in the above chart, that means you are eligible to earn My Health Pays® rewards.
- Call Member Services and ask if your plan is eligible to earn rewards.

A carousel is a rotating set of images, rotation stops on keyboard focus on carousel tab controls or hovering the mouse pointer over images. Use the tabs or the previous and next buttons to change the displayed slide.
Remember to keep your Visa Prepaid Card after you use it. As you earn more money, it'll automatically be added to your card.
Your rewards are good for a full year. Funds expire 90 days after termination of insurance coverage or 365 days after date rewards were earned, whichever comes first.
Requirements for Earning Rewards
All rewards are limited to one per calendar year.

Vaccine must be received during flu season (September thru March).
Rewards can be earned for completing an HbA1c test - $20 or retinal (eye) screening - $20.
Your reward will depend upon the type of screening your doctor recommends. Only one test per year will be rewarded. The various screening types and rewards are as follows:
- Colonoscopy
- Flexible Sigmoidoscopy
- Fecal Occult Blood Test (FOBT)
- Fecal Immunochemical Test (FIT)
- Stool DNA test
